Create custom Single Imprinted Prescription blanks.
Layout format BO prescription blanks are imprinted with the doctor’s name, address, phone number, DEA Number, and other information as space permits at the top and pharmacy advertising at the bottom.

Single Imprinted Prescription blanks are printed on 20lb white paper in black ink.  Prescription blanks measure 5 ¼” x 4 ¼” and are in pads of 100.

Ordering is as simple as 1,2,3.

1. Choose your quantity
2. Choose your signature area format
3. Enter the imprint information for the top of the prescription and imprint information for the bottom of the prescription

Additional available options:

• Colored paper is available for $3.00 per thousand extra (Pink, Blue, Canary, Green)
• Prescription blanks may be printed taller-than-wide for no additional charge.
• Other ink colors are available for $15.00 per thousand extra.
