Create custom Single Imprinted Prescription blanks on colored paper.
Layout format BO prescription blanks are imprinted with the doctor’s name, address, phone number, DEA Number, and other information as space permits at the top and pharmacy advertising at the bottom.

Single Imprinted Prescription blanks are printed on 20lb colored bond paper in black ink.  Prescription blanks are 5 ¼” x 4 ¼”.
Paper colors available: canary, blue, green, or pink
Printed in black
Pads of 100 (10 pads per 1000)
Ordering is as simple as 1,2,3.

2. Choose your signature area format
3. Enter the imprint information for the top of the prescription and imprint information for the bottom of the prescription
