Layout format DR prescription blanks are imprinted with the doctor’s name, address, phone number, DEA Number, and other information as space permits.

Tamper Proof Duplicate Imprinted Prescription blanks are printed on light blue and canary carbonless security paper in black ink.   Erasures will be obvious and if a prescription is copied the word “VOID” will appear across the copy.  Prescription blanks are 5 ¼” x 4 ¼”.
