Create your own custom Duplicate Imprinted Prescription blanks
Layout format DS prescription blanks are imprinted with the pharmacy’s name, address, phone number, DEA Number, and other information as space permits.

2-part Imprinted Prescription blanks are printed on white and canary carbonless paper in black ink.  Prescription blanks are 5 ¼ x 4 ¼

Ordering is as simple as 1,2,3.

1. Choose your quantity
2. Choose your signature area format
3. Enter the imprint information for the top of the prescription

Other options:

Colored paper is available for $3.00 per thousand extra (Pink, Blue, Canary, Green)

Prescription blanks may be printed taller-than-wide for no additional charge.

Other ink colors are available for $15.00 per thousand extra
