Create custom Triplicate Imprinted Prescription blanks.
Layout format DS prescription blanks are imprinted with the pharmacy’s name, address, phone number, DEA Number, and other information as space permits.

Triplicate Imprinted Prescription blanks are printed on white, canary, and pink carbonless paper in black ink.  Prescription blanks are 5 ¼” x 4 ¼”.

Ordering is as simple as 1,2,3.

2. Choose your signature area format
3. Enter the imprint information for the top of the prescription

Other available options:
Prescription blanks may be printed taller-than-wide for no additional charge.
Other ink colors are available for $15.00 per thousand extra.
